Patricia A. HernandezPatricia A. Hernandez, 66, of 5313 Greenbrook Drive, Portsmouth transitioned Thursday, July 30, 2015 at Kindred Transitional Care and Rehab-Nansemond Pointe. She is the daughter of the late Louise Colbert Jolly and Samuel B. Cheeks. She was a graduate of John F. Kennedy High School Class of 1967. She leaves to cherish her memories her husband, Ramon Hernandez; five sons, Demetrius Holland of Jacksonville, FL, Troy Holland of Suffolk, Michael Holland, Ranaldo Hernandez and Ramon Hernandez, Jr all of Portsmouth, VA; brother, Samuel Cheeks, Jr (Connie), of Suffolk; devoted cousin, Gloria J. Colbert of Suffolk; 5 grandchildren; numerous cousins, family and friends.A graveside service will be held noon, August 4, 2015 in Carver Memorial Cemetery.
